United Healthcare/Oxford - When selecting plan, choose Liberty, Metro or Freedom network depending on plan you are choosing.  For those searching providers in the Liberty Network for Out-of Area coverage click here and scroll down and choose the Core Network. For those searching for out of area providers in the Freedom Network scroll down and choose the Choice Plus network.

Anthem BC/BS – Select a plan for basic search. Find out if a doctor, hospital, or other care provider is in-network for the plan selected. Select the type of plan or network – choose medical plan. Next choose New York, then select how you get health insurance? – Medical (employer sponsored). Under Select a Plan or Network – choose either Blue Access Network Anthem EPO, Connection Network Anthem EPO, or PPO/EPO Network Anthem EPO or PPO. When searching for out of area coverage select the National BlueCard PPO Basic network.
